I've got about a month or two worth of total in game days played now, so I'm not certain, but I haven't struggled with infection yet. Whenever I get a scratch/laceration I either immediately put a sterilized bandage on it, or if i don't have any on me I put a clean one on and sterilize more on the fly or go back to base to prioritize treating the wound before infection can set in.
